Physician offices hire most of them. Hospitals and outpatient centers hire them too. A one-clinician office feels the empty chair the same day, because the person left rooming patients is the clinician.
The certificate argument goes wrong when someone treats CMA, RMA, CCMA, or NCMA as a state license. Those are national credentials. Washington credentials the role. California leaves medical assistants unlicensed and still writes down the tasks. Florida defines the job in the medical practice act and uses certification for the certified title, not as a license to walk in.
The work
The morning is the waiting room and the rooms behind it. You pull the chart, take blood pressure and weight, and ask what the clinician will not have time to restart. Then you stock, sterilize, and answer the call that is really a refill request.
Clinical tasks depend on the state and on what the supervising clinician will put their name on. Scheduling, insurance forms, and supply orders are the half of the job that still has to happen when the last patient is late. Some assistants do both halves. Some offices split them.
A good shift is quiet in the places that matter, because the vitals match the person in the room and the specimen label matches the order. The note in the chart is what the patient said, not the shorter version you were rushing toward.
License and training
Most states do not issue a medical assistant license. Employers often want a national certificate anyway, and the state still decides which tasks a clinician may delegate. BLS tells readers to check the state because a few places require a credential, a training program, or both. This page states a rule only where a statute or board page was read. If your state is not listed, call the medical board before you assign an injection or a blood draw.
California
California does not license medical assistants. The Medical Board says they are unlicensed people who do routine technical support in an office or clinic, with the supervisor on the premises, and they may not be used for inpatient care in a general acute hospital. Injections, skin tests, and blood draws wait on the training in the regulations, and starting an IV is off the list.
Florida
Florida defines a medical assistant in section 458.3485 as someone working under a physician's direct supervision. The listed duties include vitals, venipuncture, nonintravenous injections, and medication the physician directs. Certification from an approved program is how you earn the certified title. The statute does not say the certificate is a license you must hold before the first day.
New Jersey
New Jersey's rule 13:35-6.4 lets a physician direct a certified medical assistant to give intradermal, intramuscular, or subcutaneous injections and to perform venipuncture, in the limits that rule sets. The rule's definition wants a postsecondary program of at least 330 clock hours and a current certification from a body the board accepts. An uncertified assistant is not who that rule is written for.

Washington
Washington requires a Department of Health credential before someone practices as a medical assistant-certified, medical assistant-registered, medical assistant-phlebotomist, or the other titles in RCW 18.360. A certified assistant needs an approved program and an approved exam. A registered assistant is tied to the endorsing employer, has a narrower scope, and cannot perform phlebotomy.

Is a CMA or RMA a license?
CMA, RMA, CCMA, and NCMA are national certificates from different bodies, not a state license. Most states do not license the occupation at all. Washington does credential it, and New Jersey's injection rule is written for a certified medical assistant, so bring the certificate and still read the state line.
Is this job clinical or clerical?
Often both, and the mix depends on the office. BLS describes vitals, histories, scheduling, specimens, and injections only as state law allows. A posting that says medical assistant and means only the phones is a different job. Ask which half they will actually hand you.
What do people do after medical assisting?
Some stay and run the office. Some move into nursing, a lab program, or a specialty clinic that wants the same rooming skills with a narrower set of procedures. None of those moves is automatic. The certificate you hold now does not convert into an RN license or a phlebotomy certificate the state requires.
What should a one-doctor office check before hiring?
Check the tasks your state allows, then ask for proof they have done those tasks under supervision. Confirm who is on the premises when an injection happens. A certificate from a school last year is weaker than a year of rooming in an office like yours, and both can be true of the same person.
